Foster Home Application for birds Last Chance at Life All Breed Animal Rescue and Adoptions Foster Home Application for birdsThis form is designed to provoke thought about whether or not you are prepared to offer the best possible experience to a bird in need. We expect several things of our fosters, including compassion, perseverance and reliability. Do you exemplify these qualities? Please give each question serious thought as it will help us to determine your eligibility. Sharing your home with a bird, even temporarily, is a responsibility to be treated with care and respect. Many of our birds will require extra loving care as they recover from illness or learn correct behavior. If you are up to the challenge and ready to reap the rewards of helping to save the life of a bird, apply today! A Foster Home is a temporary home for bird until our organization finds a permanent home. If you are interested in potentially providing a permanent home for one of our adoptable birds you should fill out an adoption application instead. During the adoption process, the adoptive home is allowed a predetermined trial period to determine if the bird is a match for the home. The majority of our birds come to us from shelters, humane societies or other rescue groups. Because of this we have no history of the animals medical or behavioral past or needs. We have begun this process and will update you as to where we stand with each bird you are interested in fostering. Thank you for offering to become a foster home for our organization! To be considered as a foster home for our organization, you must: Be 21 years of age or older. If you rent your home you must have consent from your landlord to have a bird. Have all dogs and cats in the home spayed or neutered, if they are 6 months or older, unless there are medical reasons for not doing so. Have all pets in the home up to date on vaccinations. Agree to have a home visit done by one of our volunteers. The volunteer will need to see the areas of the home where the bird will be housed and also visit with family members and current pets. You must also agree to the following conditions, which are included in the foster home agreement contract: Agree to have the foster bird reside in the home, not outside. The foster bird may NOT be left outdoors. Agree to never allow foster bird to be left unattended with young children. Our rescue organization is an all (unpaid) volunteer organization. For each application we receive it takes us approximately 3 hours to review, contact all personal and veterinary references, and make a decision on approval of it. Depending upon volunteer availability, this can take us several days to accomplish, especially when we receive many applications or if we have difficulties reaching references. If the animal cannot be kept by the foster family, the foster family shall notify a Last Chance at Life Rescue Coordinator immediately. The animal shall not be sold, abandoned, relinquished to a pound or shelter or to a Rescue group other than Last Chance at Life, transferred, or given to any individual, or to any retail or wholesale establishment. We appreciate your desire to help our Rescue by fostering one of our adoptable pets, but in order to ensure the safety of your foster we ask that you review and sign the following terms and conditions. If for any reason you have to return your foster you must return him/her to a Last Chance at Life representative. but it is past 7PM on a weekday or 4PM on a weekend you must keep your foster until the next day when a representative is available. We cannot always guarantee transport! We also ask for a 2 week window for long term fosters unless in the case of an emergency or prior agreed upon ending date. Last Chance at Life must approve all adoptions; you are not to give your foster to anyone without the approval of a Last Chance at Life representative. I am aware that the educated opinions of the rescue volunteers do not constitute a claim or guarantee as to the temperament, health or mental disposition of any animal available for foster or adoption. I herby accept possession, legal guardianship and responsibility for the animal I am fostering and discharge Last Chance at Life forever from liability for any injury or damage to any person or property caused in the future by said animal, and from any causes of action, claims, suits, or demands whatsoever that may arise as a result of such injury or damages.
